As far as I know there are two possibly outstanding bugs (https://github.com/angband/angband/milestone/11) - duplicate uniques and duplicate artifacts - and I have been unable to pin them down...

There are two different things, mushrooms...

There are two different things, mushrooms (object) and mushroom patches (monster). If you're attacking it, it's a mushroom patch and you can't pick it up, only kill it.

Yep, it was a problem in the nightlies, introduced when I was trying to fix the bug which has allowed duplicate artifacts. Now fixed.
