A Flask of Chaos
This Flask weighs 1.0 lbs. It is balanced for throwing. When applied to coat an arrow, bolt, sword, or polearm, it hits to blast one target with chaos for 31-30 damage...

Went something like this, roughly:
7th level Ranger at 50 ft of dungeon:

You have no more scrolls titled "neep pro" of Summon Monster.
The Nether Wraith touches you.
You feel your life force draining away!
You have forgotten the spell of Phase Door. (doh!)
The Nether Wraith touches you.
You die.

Since that episode, I always read unidentified scrolls on a staircase.
