this exists in my variant, DaJAngband:

F:EVIL
S:1_IN_2
S:CAUSE_3 | SCARE | TRAPS | S_APE
D:Green skin, crooked nose, pointy hat, she's got the classic witch look, but not every
D: witch has winged monkeys..

and yes, winged monkeys are among the apes that she can summon.

BTW: The NEVER_MOVE flag makes the KILL_WALL, TAKE_ITEM, and open/bash door flags do nothing.

Uhm, I've never really checked - does a monster attack with all attacks each turn (that would be 4 blows for this one)?

Yes, one melee attack set for each of its turns.
