Never noticed it either. How exactly is it tied to damage? Is it something that will mostly affect only melee-heavy characters, given how the new breath mechanics work?
It becomes possible if the damage is close to the maximum for a resisted breath - for example, for fire the effect happens if a random number between 0 and the damage is greater than 500.

Can we get customized additional messages to explain why the side-effect happened? Since it's rare, I suspect players will otherwise be left confused as Ingwe was.

Currently the tile @ is standing on can get webbed or turned to lava without the player being aware of it. Can we perhaps get terrain showed in the status line?

Yes, that would help. This actually was the case in my compo death 1h ago. Didnt realize i was on lava when I died

I agree this is a problem, but at least the game now prints a message every turn ("The lava burns you!").
