Yup, the Louse. HT warrior level twenty-something... or maybe it was the hunger that got credit. Either way if the breeders hadn't stuffed the hall so effectively he'd not have been stuck there...

My early adventurers die like flies. I lost a level 1 adventurer before to a green mold killing me with poison. I was definitely not expecting to die from full to poison, but that's what happens when you have 12 hp.

Not quite in the spirit of the question, but something off-screen breathing nether at me. Or Wiruin, who doesn't show up on ESP, so I'm always surprised when he kills me.
Or, even further from the spirit of the question, how about Osse? We're both trying to kill Morgoth, if I didn't know better I'd sure as hell be surprised that this guy is attacking me. |

Along with the Q (greater ones and that nexus one) monsters, Wiruin is one of the few monsters I'm actually scared of, when running an endgame character. |

i love louses, fleas, red worm masses, and to an extent, black oozes. Get yourself a nice long corridor, and you can jump to CL20 in no time.

Ossė. I swear every turn he leaves you stunned. 3(!) different water spells, and he picks them almost exclusively.

Battle scarred veterans. No part of the game is as terrifying as your first visit to town as a level 1 mage. Leads to such tense situations as "Crap, there's a battle-scarred veteran next to the wand shop, but I *really* want to see if they have any wands of magic missile for sale. Do I try to sneak past the slumbering murder hobo, or should I just head into the safety of the dungeon?"
