In that quest monsters will be steadily populated on every level you are on, not just the quest level. You might be better off switching to the latest beta of NPP 0.5.4, or at least incorporating the patch that fixes that particular bug (on github).

Rienya? That's because it is a randart based on an existing artifact generated at startup. You can tell by the activation (I have not yet given in-game generated artifacts activations). From the looks of it, it is probably based on one of the paur* artfiact gloves. So it probably has a very low rarity and native depth, but it just took a while for the game to pass the rarity roll and generate the artifact.
