From what I can tell from this dump, you need to run away earlier. Once you can be killed by a normal attack or two at the depth you are at it is too late to safely teleport. If you do teleport into danger you should tele again. Also, I think you will find things easier if you drop a bit faster- spending 1.3 million turns getting to dlvl 30 just invites bad luck to kill you.
