For the rest, are you sure you killed them the first time and that they didn't teleport away or something? Alternatively, did you read the Crumpled Scroll of Mass Ressurection?

There are very limited circumstances under which uniques can appear twice, but its pretty rare and very unlikely to happen at all, much less multiple times during the same game, and it don't think it applies to Lokkak the Ant Queen.

Finding the player corpse is pretty neat, but really is only useful to possessors who have lost their original body one way or another.
